PHARMACODYNAMICS:

ZIDOCIN DHG is a combination of spiramycin, an antibiotic belonging to the macrolide series and metronidazole, an antibiotic belonging to the 5- nitro-imidazole, used in the treatment of bucco-dentai infections. Spiramycin has antibacterial activity vis-a-vis the microorganisms usually found in bucco-dentai infection such as streptococci non-D, Pneumococci, Mycoplasma. Chlamydia. Corynebactenum. Achnomvcetes The antibacterial activity of metronidazole vis-a-vis microorganisms usually found in bucco-dentai infections is as follow: strict anaerobic bacteria Clostridium, Bacteroides fragilis, Peptostreptococcus. Peptococcus, C. perfringens. Bifidobacterium bifidum.

PHARMACOKINETICS:

Zidocin ( Spiramycin ) is incompletely absorbed from the gastrointestinal tract. Drug absorption is approximately 20 – 50% of the dose. The mean half-life is 5-8 hrs. Spiramycin is mainly eliminated in bile. Metronidazole is rapidly and completely absorbed after oral administration.

Elimination half-life is about 8 hrs. Metronidazole is metabolized in liver under hydroxide and acid forms, eliminated via urine partly under glucuronide form The two constituents of ZIDOCIN DHG are well concentrated in stomatological tissues: salivas, gingival and alveolar bone.

INDICATIONS:

Treatment of acute, chronic or recurrent stomatological infections, especially dental abscesses, gingivitis, paradontitis, stomatitis, parotiditis, submaxillitis. Prophylaxis of postoperative stomatological infections.

CONTRAINDICATIONS:

Hypersensitivity to spiramycin or metronidazole. Children aged less than 6 years.

PRECAUTIONS:

Zidocin should be used with caution in patients with hepatic dysfunction because it may cause hepatotoxicity.

It is unadvisable to take alcoholic drinks during the treatment. The blood formula should be monitored in case of long-term treatment.

PREGNANCY AND LACTATION:

Zidocin should not be used in the first trimester of pregnancy. Stop breast-feeding while taking ZIDOCINDHG.

VEHICLE DRIVERS AND MACHINERY OPERATORS:

Cautions should be taken when driving vehicles and operating machinery.

INTERACTIONS:

Related to spiramycin: concomitant use of spiramycin with oral contraceptives will become ineffective contraception.

Related to metronidazole: metronidazole enhances the effect of anticoagulant (warfarin) and muscle relaxant drugs without depolarization (vecuronium). Concurrent administration with alcoholic drinks may cause disulfiram-like reactions. Phenobarbital may accelerate the metabolism of metronidazole.

ADVERSE EFFECTS:

Gastrointestinal disorders: Gastric pain, nausea, vomiting and diarrhea.

Allergic reaction: Urticarial rash.

Related to metronidazole: Metallic taste, glossitis, stomatitis, mild hypoleukemia, brown-color urine.

Inform your physician about any adverse effects occur during the treatment.

OVERDOSAGE:

For spiramycin: No documents have been reported.

For metronidazole: single oral doses of metronidazole up to 15 g have been reported. Symptoms reported include nausea, vomiting, and ataxia. Neurotoxic effects, including seizures and peripheral neuropathy, have been reported after 5 to 7 days of doses of 6 to 10.4 g every other day.

Treatment; There is no specific antidote for metronidazole; therefore, management of the patient should consist of symptomatic and supportive therapy.

DOSAGE & ADMINISTRATION:

Adults: oral dose of 4-6 tablets daily in 2-3 divided doses.

In severe cases, the dosage may be increased to 8 tablets daily.

Children aged trom 10-15 years: oral dose of 3 tablets daily. children aged trom 6-10 years: oral dose of 2 tablets daily.
